Smart Therapycollect two types of information. These include:

This information is necessary for services and billing. Your personal information is securely stored at all times in accordance with HIPAA rules and regulations.
  2. Information We Collect From Your Use of Our Services: We collect information about what services you use and how you use them. This may include but is not limited to cookies, referral sources, client demographics, and website traffic.

How This Information Is Used

The information collected is used for providing client services (i.e. insurance billing) and for continuing to better our services by monitoring the needs and choices of our consumers. Your privacy is very important to us. We make sure that your personal information is securely stored in accordance with HIPAA rules and regulations.

Information We Share

Except as follows, Smart Therapy will not disclose any personal information without your permission. Your personal information may be shared with third parties (such as credit card companies) in order to a) provide client services, b) collect payments, c) comply with valid legal requirements such as a law, regulation, search warrant, subpoena or court order or d) in special cases, such as a physical threat to you or others. In the even that we are legally compelled to disclose your personal information to a third party, we will notify you unless doing so would violate the law or court order.

Protecting Your Personal Information

Smart Therapy takes reasonable technological and physical security measures to protect clients' personal information. Personal information is accessible to a limited number of Smart Therapy employees and staff who are given a secure password to gain access to such information. While reasonable measures are taken to protect your personal information, Smart Therapy cannot guarantee that a breach of security will not take place and will not be held liable. Should such a breach of security occur, Smart Therapy will make a reasonable attempt to notify you by mail and/or email to the best of their ability.
